Camper Forms & Questionnaires
We are excited to offer you two choices as to how to complete your campers pre-camp forms. Whichever format you choose, please remember that we need all forms returned at least two weeks before your camper’s session begins. Having forms in advance gives us plenty of time to review them and makes checking in much smoother and easier for everyone involved.
We would prefer all camper paperwork to be completed electronically, however if you would like to use the paper forms and scan and email (to registrar@ymcanwnc.org) or fax (336 983 4624) or mail them (1225 Camp Hanes Road, King NC 27021). Please be sure all forms are received in the office at least two weeks before your camper’s session begins.
